When exploring new ways or challenging everything or trying to smell out issues it many be helpful to remember several things about the notion of process improvement in general
If we've identified a problem and we're looking for a solution we need some way to test that solution against the problem. If we know these ways, we need to compare them to each other and the problem. If we don't know the solution(s) we need to come up with a list of possible solutions and test them in some way. In the absence of testable solutions, no progress can be made to solving the problem or improving our situation.
Any ideas put on the table need to have some way of being tested for validity, the benefits of the solution, and the effectiveness of any proposed solutions.
In the absence of these activities it's just wandering around observing problems and suggesting others should discover the solution themselves - a sort of self-actualizing process.
