One source of project failure in any domain is the failure to have the answer to What Does Done Look Like? The answer to this question has to be in units of measure meaningful to the decision makers. Here's a white paper on this topic.
As always the answers to each of the questions are asked in the presence of uncertainty about future outcomes and because of that the answer needs to be estimated in some way. Without making those estimates, it's not possible to have any visibility into the outcomes of the decisions that are made along the way to Done.
