There are several realities in our complex software-intensive system of systems
- All risk comes from uncertainty. Uncertainties come in two forms that can be handled
- Aleatory uncertainty that is handled by margin
- Epistemic uncertainty that is handled with buy down activities
- No risk-informed decision can be made without estimating the risk and the impact of the handling strategy
- No value can be determined until you know the cost to produce that value
- Those paying for your work get to say what is waste or unneeded. Not those spending that money
- Without knowing the Root Cause of the observed dysfunction, no corrective or preventive action has a hope in hell of being effective
